Here's what this system does

If you are injured on the job, or if you are an employer and an employee is injured on the job, you are normally required by state law to file a First Report of Injury (abbreviated 'FROI') with the state department that oversees industrial safety. (In California, for example, this is the Division of Workers' Compensation, part of the Department of Industrial Relations.) Normally, you report an 'incident' to the insurance company that provides the workers compensation coverage for the employer. The insurance company handles the reporting to the state as part of the process of handling any 'claim' for workers compensation benefits. This system shows how the process of workplace injury & claim reporting can be made easier and more user-friendly, as programs like TurboTax and TaxCut make tax reporting easier. Note: this system is currently not connected to anything, so please do not try to use it to report a real injury! You are welcome, if you like, to use it to generate a real paper form that you should be (no guarantees) able to submit to your real insurance company.
